Return to Athletics Plan

Superintendent Addison Davis has released his Return to Athletics Plan which details the safety guidelines for our student-athletes and spectators.

Each fall participant will have the opportunity to purchase up to 4 tickets for each contest.  The details are included in the Plan.

Middle School Sports are specifically addressed in the Plan.

We also encourage you to watch this video illustrating the spectator experience.

Student Participation Requirements

EL2 Physical Form - Completed, which includes: signed, stamped, dated, and cleared without limitations by approved medical personnel)

Three (3) required FHSAA Videos - Concussion in Sports, Sudden Cardiac Arrest, and Heat Illness Prevention (Print all three certificates in student-athlete's name, dated after May 15, 2019)

Purchase Insurance - Print insurance card

Government-issued Photo Identification of Parent/Guardian who is signing the forms for the student-athlete.

Once all of these documents are collected and ready for  upload, please complete the online registration for your student-athlete at www.planeths.com
