ABSOLUTELY NO CONTACT IS TO BE MADE WITH ANY PERSONNEL THAT ARE CONNECTED TO OUR ATHLETICS PROGRAMS UNLESS STUDENT IS ENROLLED AT PLANT HIGH SCHOOL. PERSONNEL INCLUDES AND NOT LIMITED TO PARENTS STUDENTS BOOSTER MEMBERS AND THOSE AFILIATED WITH ATHLETICS AT PLANT.

THE 2025-2026 UPDATED DIRECTIONS WILL BE POSTED ON MAY 15TH 2025.

NEW: STUDENTS MUST WATCH THE SPORTSMANSHIP VIDEO AND IT IS NOW A PART OF THEIR CLEARANCE PAPERWORK. THE COURSE CAN BE FOUND ON NFHS LEARN. VIDEO CERTIFICATE MUST BE DATED MAY 15TH 2025 OR LATER.

CLEARANCES FOR ATHLETICS DOES NOT OPEN UNTIL MAY 15TH 2025. THERE COULD BE A SLIGHT DELAY AS ATHLETIC CLEARANCE IS LOCATED IN CALIFORNIA AND THE TIME DIFFERENCE SHOULD BE TAKEN INTO CONSIDERATION. 

Student MUST be enrolled at Plant High School to make contact with anyone that is affiliated with the Athletic Department (FHSAA Policy 36) and to be able to submit their clearances. No contact can be made with anyone associated with Plant High School Athletics until a student is enrolled. 

We are NO longer accepting or processing any clearances for 2024-2025 school year.

PLEASE READ ALL OF THE DIRECTIONS BEFORE COMPLETING THE CLEARANCE. 

You MUST submit a clearance for EACH sport that you plan on trying out for, EACH school year. This means you need a new clearance for the 2025-2026 school year.  

**If you have a current physical (EL2) that has not expired - you may upload that EL2. Both pages must be uploaded.  Any physicals completed after April 15th 2025 should be on the new EL2 form below. **

Expired EL2 Procedures: If your EL2 (physical) expires during the school year, you will need a new one. Please take the new EL2 form (can be downloaded from link below) to the doctors office and get a physical. Once completed - please REPLACE your old EL2 with the new one.  Once you have uploaded the new one (see upload tip below and don't forget to save and submit at the bottom of the upload page) please email Ms. Omensetter to let her know so she can process. evanitta.omensetter@hcps.net. You MUST have a current unexpired EL2 to participate and be eligible. Physicals (EL2) are good for 365 days.

Clearance Tip: Gather ALL the things that you need prior to logging in and uploading.  If you have all the items - the process is much easier using the upload tip below. 

Baseline Concussion: Available for students to complete. Please see directions below.

Upload Tip: Log into your athletic clearance account using your smartphone. Find the sports clearance and click on the circle that says files. Find the file that you need to upload and click on browse. This will give you an option to take a picture - click on it. Take a CLEAR picture of the ENTIRE page. DO NOT cut off edges, click on use photo. Allow for it to upload. If you have multiple files to upload - repeat the process. Once done - scroll to the bottom and click on save and continue. If you do not click on save and continue, the files won't save and they won't submit for clearance.
