    Please complete the application and bring it to the Hillsborough County Public School Head Start site of your choice along with the documents below:

    Bring the following items to complete your application:

    1. Copy of Child’s Birth Certificate

    2. Earned Income

    • 1040 Tax Return or 1099 Form

    Other Income (Last 12 months printout)

    • Cash Assistance (TANF/AFDC)
    • Child Support Verification
    • Veteran’s Benefits
    • Financial Aid Award Letter
    • Unemployment Compensation/Worker’s Compensation
    • Social Security Administration (SSI, SSB, or SSD award letter)
    • Proof of Residence (one of the following):
    • Florida Driver’s License/ID with current address
    • Utility Bill or Pay Stab within the last 60 days
    • Rental Agreement
    • Military Order
    • Other Documents
    • Proof of Legal Custody/Guardianship
    • Food Stamps/Medicaid Eligibility Letter (Optional)
    • Health Documents Needed for Enrollment:
    • Current Physical with Hemoglobin & Lead Results
    • Current Immunization Record (Shot Record)
    • Current Dental Exam
    • Medicaid Card/Letter must be provided after your application has been determined eligible and approved for enrollment.
